A senior nurse in the department approached you to discuss about her concerns regarding a new junior registrar. She has received numerous complain from the nurses that the doctor has been very rude and demanding when giving treatment orders to them. One nurse said he made sexually inappropriate comments to her. Some nurses have refused to be on the same shift as the doctor.

Instructions for the candidate

You have made an appointment to see the senior nurse in your office after the initial meeting. How would you approach and manage this situation?

For the actor

You are the nurse manager of this ED for the last 4 months and you are still trying to adapt to the system. The department is under a lot of pressure with bed block, increasing number of presentations and an increase in nurse turnover. The last thing you need is a badly behaved doctor scaring away your nurses. You are determined to get this doctor out of the department to avoid more nurses leaving. You are willing to work with this consultant to resolve this issue

The doctor has been rude, derogatory, disrespectful and even sexually inappropriate with one of the nurses. There are multiple nurses complaining about multiple events.

- Take complain seriously
- Clarify issues
- Collate evidence – talk to nurses involved, record details
- Senior staff to observe doctor
- Check with previous employers and AHPRA regarding previous problems.
- Check mental health history
- Mindful of cultural issues
- Inform administration/risk management
- If substantiated, arranged meeting with doctor, at least two consultants, documentation made
- If doctor acknowledges bad behaviour, work out acceptable parameters and follow up.
- If doctors denies bad behaviours, informed him of complains and will be managed.
- Arranged follow up date.
