Scenario:

You are the consultant rostered on a non-clinical shift. A trainee who was recently involved with a difficult case (a patient presenting with thyroid storm) has asked you to discuss this rare and difficult condition's diagnosis and management.

Instructions for the candidate

You are to discuss with the trainee the diagnosis and management of thyroid storm.

For the actor

You are an ACEM provisional trainee who has recently been involved with a difficult case of a 42-year-old woman who presented unwell with fever, tachycardia, nausea and vomiting. She was initially treated in the ED as a case of severe gastroenteritis and managed with IV fluids, antipyretics, and antiemetics, however when the patient deteriorated/failed to improve. When the ED Consultant on shift became involved it was discovered that the patient was suffering from thyroid storm.

You had never before seen a case of thyroid storm and did not fully understand the diagnosis and management. You are to request a brief tutorial on the topic so that you are more prepared in the future.
